For people outside Victoria, meetings and routine coordination can generally be completed remotely. Any local in-person services must be arranged with suitable providers in the person's area. Availability and the final service model are confirmed during enquiry.

Support at Home and case management

Australia's Support at Home program provides assessed in-home aged care through approved plans and service arrangements.
